Case Manager
To plan, coordinate and implement program services to assist clients to obtain optimal functioning.
Duties and responsibilities

  • Manage a client caseload of approximately 50 clients.
  • Coordinate and document meetings on an annual, semi-annual, and quarterly basis, with client and support team members to discuss and review progress on objectives.
  • Maintain client records in accordance with Department of Developmental Services (DDS), Community Care Licensing (CCL), and Regional Center standards.
  • Review intake documents to screen prospective clients for admission into the program.
  • Collaborate within the team to develop initial service plans and program schedules for each new client.
  • Collaborate with the client, staff, care providers, and service coordinators to develop each Individual Service Plan.
  • Team with the Program Manager, Assistant Program Manager, and direct staff in developing and researching resources to schedule daily activities relative to client goals and Individual Service Plans.
  • Assist clients with concerns relating to their daily activities.
  • Act as a liaison to care providers, staff, and Regional Center service coordinators regarding client needs, wants, and desires.
  • Collaborate with other members of the Admissions Team to development, implement and monitor behavior intervention plans, vocational training plans, and community integration plans.
  • Ensure assigned time studies are executed by the due date as per Task Analysis and standards of DOL & FLSA.
  • Review IWP with worker to verify placement is satisfying individual’s goals.
  • Learn, adapt and apply, according to established milestones, changes in the use of agency’s technology as they occur.
  • Assist in reviewing and communicating recommendations of any serious incidents.
  • Assist in assuring quality services is provided to clients per the Program Design.
  • Attend staff meetings, client support meetings, and other meetings as assigned.
  • Work with staff, care providers, and other agency case managers to assess client progress, evaluate effectiveness of service plans, and adjust as appropriate.
  • Ensure coordination of program services, prepare written assessments, program plans, and evaluations of client assigned caseload.
  • Monitor client health, medical, and behavioral concerns.
  • Transport clients as needed.
  • Maintain professional standards and conduct as case responsible person.
  • Create and maintain client records.
  • Responsible for safety of all persons assigned.
  • Adhere to all safety regulations. Be aware of and report safety concerns.
  • Provide coverage to assigned back-up case manager in his/her absence.
  • Train and onboard new staff as directed by Program Manager.
  • Perform all other duties and responsibilities as assigned.

Qualifications and Skills

  • Preferred B.A. degree in an appropriate field or equivalent, with a minimum of one years’ experience in rehabilitation or related field and demonstrated knowledge of behavioral technology.
  • Oral and written communication skills required
  • Current CPR and First Aid training required.
  • Must have a clear driving record, valid CA driver’s license, 5 years driving experience, safe reliable transportation, and proof of current vehicle insurance.
  • Able to lift up to 25 lbs. by self; up to 150 lbs. with one other person; able to push wheelchair with consumer up to one mile; standing walking, sitting, pushing bending, pulling required; squatting and repetitive motion may be necessary.
  • Proper handling/use of chemicals for cleaning and work crews may be necessary, use according to manufacturers labeled instructions.

Job Type: Full-time

Pay: $20.19 per hour
