The Community Health Worker serves as part of the IAP program. Under supervision, the Community Health Worker will implement COVID-19 community outreach and health education efforts to residents of the following service area cities/towns: Hartford, East Hartford, Manchester, Meriden, Middletown, and Windsor. Works with program staff, community partners, stakeholders, and other agencies throughout the service area to promote vaccine confidence, increase vaccine uptake, provide data-driven COVID-19 information, and address vaccine misinformation. Conducts community canvassing, tabling events, and door-to-door outreach. Provides in-person and/or virtual COVID-19 educational sessions and presentations. Maintains detailed outreach data and prepares programmatical reports. Attends community vaccination events, building relationships with community partners. Performs related work as required.

The hours of work are 35 per week. Required to travel to site locations occasionally.

This is a grant funded position. Position is contingent on grant funding.


Knowledge, Skills, and Abilities

  • Knowledge of Community Health practices, including facilitation of presentations and data collection
  • Knowledge of technical platforms (Microsoft Suite: Excel, Teams, etc.)
  • Ability to communicate effectively, clearly, and concisely both orally and in writing
  • Ability to display provide excellent care services to behaviorally challenged clients
  • Ability to work flexible work hours including pre-scheduled nights and weekends
  • Strong organizational skills

Qualifications

High School Diploma or GED and two (2) years of experience providing education, direct care services or closely related field to a diverse population.

Bilingual – Spanish/Portuguese required OR Be a certified CHW

Must have and maintain a valid Driver’s License

Preferred Qualifications:


§ Associate’s or Bachelor’s Degree in Social Service, Human Services, Public Health or related field
