Overview
Good Samaritan Home Care has provided home care and wellness services for over 29 years. Good Samaritan Home Care’s quality care is provided through a team of registered nurses, social workers, aides and physical, occupational and speech therapists.
Responsibilities
- Coordinates care with caregivers and physicians to improve and/or maintain the social, emotional, functional, and physical health status of the patient.
- Assists the physician and other caregivers in understanding the significant social, emotional and economic factors influencing the patient's ability to improve medically.
- Provides resources and education to patients and their caregivers
- Participates in the coordination of services with other disciplines and community resources as needed
- Evaluates and revises care plans/assessments based on changes in patients and/or their environments
- Participates in regular case conferences and in-services or special case orientations as needed
Qualifications
- Master’s degree from an accredited school
- Currently licensed to practice in the state of Indiana
- Must obtain and maintain current CPR Certification (American Heart Association or American Red Cross)
- Valid drivers license, car insurance and reliable transportation
- One year of social work experience in a health care setting required
Pay Range
$20.92 - $28.78 /hour
